Please note for the vast majority of patients you will be able to access proof of vaccination via the NHS app (which is also valuable for accessing your health records and ordering repeat prescriptions)
The NHSapp can be downloaded for iPhone or for android.

Alternatively you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.
The NHS Covid Pass will only show vaccines given in England. If you have been vaccinated elsewhere this will not show in the NHS Covid Pass.
The NHS COVID pass pulls the vaccine information from NIMS - (NIMS (National Immunisation Management Service) which is where the vaccination sites record the administration of vaccines. NIMS then sends the vaccination information out to the individual GP systems - it does not work in the reverse - so if we update your record on our clinical system it will not show on the NHS Covid Pass. We understand that going forward there will be a possible solution so that vaccines given abroad will show in the NHS Covid Pass.
We can provide a 'to whom it may concern letter' at a charge of £27.50 with the vaccine information (5 working days) but you need to ensure that this will be acceptable with whom ever is requesting proof.
Please do not write any symptom queries on this form. Any symptom queries or request for appointments via this form will be discarded. To make an appointment either book online via the NHS app/Patient access or by calling reception
01926 331401. We will start to process your form by the next working day but it will take up to
5 working days to complete. Please DO NOT call before
5 working days as you will not receive a status update from reception or the administrative team.
If you are NOT a registered patient then your form will be discarded. This service is only for patients registered at the surgery aged
16 and over; and for parents or legal guardians of children. Forms must be completed in the UK due to GDPR regulations.
