Reception: Low Folate/B12 Information Form - Clarendon Lodge Medical Practice

If you did NOT receive a text explaining that we found a low folate (folic acid) and or B12 level in your recent blood test then please ignore this page and DO NOT complete the form below.

As outlined in your text we have found that your folic acid levels are low. The main cause of folate deficiency is poor intake. We only store a small amount of folate in our bodies there is it important we maintain a good dietary intake.

Good food sources of folate include broccoli, Brussels sprouts, asparagus, peas, chickpeas and brown rice. Other useful sources include fortified breakfast cereals, beans, some bread and some fruit (such as oranges and bananas).

It is important you make the necessary changes to your diet as a persistently low folate can cause issues with your health including anaemia, issues with conception & the developing fetus, Cardiovascular disease and much more. More information can be found on the NHS website.

To make an appointment either book online via the NHS app/Patient access or by calling reception 01926 331401. We will start to process your form by the next working day but it will take up to 5 working days to complete. Please DO NOT call before 5 working days as you will not receive a status update from reception or the administrative team. If you are NOT a registered patient then your form will be discarded. This service is only for patients registered at the surgery aged 16 and over; and for parents or legal guardians of children. Forms must be completed in the UK due to GDPR regulations.
