Subject Access Request
Under the Genral Data Protection Regulations (GDPR), Clarendon Lodge Medical Practice is not permitted to charge data subjects for providing a copy of the requested information; this must be done free of charge. However, should a request be deemed either “unfounded, excessive or repetitive”, a reasonable fee may be charged. Furthermore, a reasonable fee may be charged when requests for additional copies of the same information are made. However, this does not permit the practice to charge for all subsequent access requests.
In accordance with the GDPR, we must respond to all data subject access requests within one month of receiving the request (previous subject access requests had a response time of 40 days).
In the case of complex or multiple requests, we may extend the response time by a period of two months. In such instances, the data subject must be informed and the reasons for the delay explained.
Information submitted through secure forms is used only for the purposes of processing your request. We may
be in touch with you in relation to the information submitted.
All Information submitted through secure forms is secured with a private key known only to the GP practice and is
accessed over a secure connection by nominated Practice staff. Our practice has a strict confidentiality policy.
This information is not shared with any third party organisations.
This information is retained for up to 28 days.
